The first step of our enrollment process is to Connect With Us by calling, emailing or filling out the form on our Contact page. We will provide more details on our services, share with you what you can expect from our center and team, and schedule a tour.
Complete our intake application form and we will work together with your child's Primary Care Physician to complete our Patient Referral Form, receive your child's prescription and confirm eligibility. We will assist you with all paperwork and will work with your insurance to get everything completed.
During your scheduled tour, you will meet our licensed medical staff who will be caring for your child and will provide the necessary skilled nursing services or therapy tailored to your child's needs. We will walk you through our entire center that will provide you with confidence knowing that your child is in a safe and loving atmosphere.
We look forward to your child joining our Superhero Kid's Team and watching them thrive and grow to their fullest potential! Once all of the steps are completed, we will schedule your child's start date with you so they can begin a new experience.
